WAIVER & CONSENT STATEMENT (Parent/Guardian)

By registering my child to participate in programs and activities offered by Canadian Youth Soccer Academy (CYSA), I acknowledge and agree to the following:

I understand that soccer and related activities involve inherent risks, including but not limited to slips and falls, collisions with other participants or objects, contact injuries, weather-related conditions, and travel to and from activities, which may result in minor or serious injury (including concussion, fractures, disability), and in rare cases may result in death. I voluntarily accept these risks on behalf of my child and authorize my child’s participation in all CYSA-related activities, including practices, training sessions, games, tournaments, camps, team events, and any related travel or attendance.

To the fullest extent permitted by law, I release, waive, and agree to hold harmless Canadian Youth Soccer Academy (CYSA), its directors, officers, employees, contractors, coaches, volunteers, match officials/referees, agents, and representatives, as well as any applicable league, district, facility, or soccer governing organizations associated with CYSA programming (collectively, the “Released Parties”), from any and all claims, demands, actions, damages, losses, or expenses arising out of my child’s participation, except to the extent such claims are caused by gross negligence or willful misconduct where such limitations are not permitted by law.

I agree to indemnify and hold harmless the Released Parties from any claims, damages, costs, or expenses (including reasonable legal fees) resulting from (a) my child’s or my own actions or misconduct, (b) failure to follow CYSA rules or staff/official instructions, or (c) inaccurate, incomplete, or withheld information provided during registration, including medical or emergency contact information.

I confirm that my child is physically and medically able to participate. In the event of illness or injury, I authorize CYSA staff or representatives to provide basic first aid and/or to obtain emergency medical treatment for my child as reasonably required, including ambulance transport if necessary, and I accept responsibility for any related costs not covered by insurance.

For safety, I agree to provide my child with proper soccer footwear and shin guards, and I understand my child may be restricted from participating if required safety equipment is missing or deemed unsafe. I agree that my child and I will follow all CYSA policies and all applicable rules and regulations of associated leagues, districts, and governing bodies. I also understand that team placement and balancing may occur up to and including the third (3rd) game of the season (or equivalent early-season evaluation period).

Media Consent: I consent to my child being photographed, filmed, audio recorded, or interviewed during CYSA activities, and I grant CYSA permission to use my child’s likeness/voice in CYSA materials such as the website, social media, and promotional or educational content. If I do not consent, I will notify CYSA in writing before my child participates and CYSA will make reasonable efforts to accommodate non-consent.

I confirm that I have read and understand this waiver and consent statement, and that I am the parent/legal guardian authorized to sign on behalf of my child.
